I have a flight from Seoul to Canada with 4.5 hours connection in Tokyo international airport. Can I go and visit city for an hour? If yes, how can I do that?

you can travel, but going to Tokyo area will take you 1 and half or 2 hrs to travel by bus, and by train using the skyline, it will take 45 minutes, but you have to ask the narita information about the details to be sure that you will not be late in your flight.
