How to mail from Canada to Japan?

I'm having trouble in understanding the postal form for letters going to Japan from another country. Does anyone know the exact form that an address in Japan should be to ensure that the letter is sent at the proper addressed person?

Japan Post is excellent at deciphering addresses.

Just do it the Western way of small to big: name, street, city, prefecture, country, with postal code at the end.

The actual way it's done in Japan though is written in Japanese from big to small, postal code at the bottom or top. Also, add "sama" after someone's name.
