Can adding か instantly form a question in Japanese?

Question:


For example, 十五才ですか?Are you 15 years old? or would that be 十五才ですね?You are 15, yes?
Would you need to use any pronouns?

Also, I don't really understand the 'and' rule. When listing a description of something or a list of things about yourself, would you use や or と?

There is no KA in it whatsoever.

That's also another phrase I use everyday with my toddler. There also is no KA in it. I change my voice to make a question, "What's that?"
